COVID-19 Policy Update

FHMS continues to take proactive steps to protect our patients and caregivers, and to prevent potential transmission of the COVID-19 virus.

With growing cases in the county, we find it necessary to reinstate our screening efforts for all in-office visits. We continue to ask all visitors to always wear a mask while in the office. To minimize risk of infection, we ask all patients attend their appointments alone unless a caregiver is necessary for young children and frail adults.

The safety of our patients and employees is our number one priority. If you are experiencing any Covid/flu-like symptoms including fever, cough, body aches, nausea, vomiting, diarrhea, or shortness of breath, we ask that you do not visit any of our offices.

Falsifying information on any COVID-19 screening forms will be considered a breach of trust and may subject a patient to termination from the practice.
