FHMS Patient Portal

The Patient Portal is an online resource that provides secure access to portions of your medical record and an easy way to communicate with your Provider. Unlike conventional email, the patient portal is completely secure allowing you real time access to your medications, test results and upcoming appointments.

You can also schedule appointments, request medication refills, pay your bill and message your Provider. To activate your patient portal, you must request an activation letter from any of our locations. It is important that you read the “Patient Portal Activation Letter” and follow the instructions carefully before you can log into the patient portal.

  • The patient portal is to be used for NON-URGENT issues only. If you’re sick and need to be seen, please contact the office. If you are experiencing a medical emergency or believe you are experiencing a life threatening situation, call 911 immediately or go to your nearest emergency department.
  • Any communication should be about you and not a family member.
  • Please remember to check your email for any updates regarding your patient portal messages, appointment and medical history.
