Online Scheduling with Family Health Medical Services

Scheduling an appointment with one of our providers is now as simple as 1, 2, 3 with our online scheduling feature! Your next FHMS visit is only a few clicks away when you follow these easy steps:

  • Step 1: Head to our online scheduling hub to get started.
  • Step 2: Answer a series of prompts as honestly as you can to ensure you get the right results and recommendations, including any virus exposures, your reason for visiting, which location you prefer, and more.
  • Step 3: Click the “Show Available Slots” button and select an appointment from the available days and times. You will be able to filter these results based on your desire location and provider.

From there, you will simply fill in your contact and insurance information before clicking “Book My Appointment.” It is that easy!

To help us prepare for your appointment, we ask that you complete our electronic pre-check-in prior to arriving at the office. A link to complete the pre-check-in will be sent to you via text and/or email. This information will be collected during the final stage of your online booking process.

If you decide to receive an email, one will be sent to your inbox after completing your booking. There is also an option to receive the pre-check-in form via text message.
