Your First Appointment

At Family Health Medical Services (FHMS) the comfort and wellbeing of our patients is our priority. We focus on caring for individuals and families, building long term relationships with our patients, and providing health education on lifestyle, chronic medical problems, and preventative health care measures.

Your first appointment with us is used to introduce you to our practice, establish your medical history, and discuss your health care needs.

What to know before your first appointment:

  • You can expect your appointment to last 30 to 45 minutes.
  • We ask that you plan to arrive 15 minutes early. This will allow confirmation of your demographic and insurance information, collect your copay, and retrieve your vitals, to ensure you are on time for your provider.
  • If you need to cancel or reschedule this appointment, please do so at least 24 hours prior. Our office has a NO-SHOW policy, and you will be charged a $25.00 no-show fee if you fail to cancel or no show this very important appointment.

If you are a new patient and you do not show up for 2 consecutive appointments, and you do not call to cancel, you will receive a call from our patient coordinator and a letter, which also will be sent to your insurance company, notifying you of your termination from the practice.

What to Bring: Medical Records

Please bring a list of your medications with their instructions to each visit. If you are taking any controlled medications, you will need to set up an additional appointment with our nurse for a risk assessment before any controlled medication will be prescribed.

During your first visit we will also discuss your past surgical and hospital history. It is necessary for you to retrieve the past 2 years of your medical records PRIOR to your new patient visit. We must receive these records at least 2 days prior to your appointment. If we do not, your appointment may be rescheduled.

Manage Your Healthcare Online, anytime.

The Patient Portal is an online resource that provides 24/7 anywhere, anytime access to your medical record and an easy way to communicate to with your Provider. Unlike conventional email, the patient portal is completely secure allowing you real time access to your medications, test results and upcoming appointments.

You will receive your patient portal activation letter in the mail. We ask that you activate your patient portal and complete your health history prior to your first appointment.
