New Partnership

 
Dr. Albert Persia has joined FHMS as a new partner along with Dr. Robert Berke and Dr. Alexander Selioutski

Born and raised in Niagara Falls, Ontario, Canada.  Dr. Persia went to University of Western Ontario for undergraduate studies and then medical school at the University of Toronto. He completed his family practice residency in Toronto Ontario in 1997. He moved to Jamestown in 1997 and worked with WCA hospital until coming to FHMS.  He has been Medical Director at Lutheran Social Services Nursing Home since 2008.  He has worked with Chautauqua County Hospice organization as Medical Director between 2008 at 2010.  He enjoys all aspects of medicine but spends a significant amount of time in geriatric care.

A patient-centered medical home is a model of care that strengthens the clinician-patient relationship by replacing episodic care with coordinated care and a long-term healing relationship. Each patient has a relationship with a primary care clinician who leads a team that takes collective responsibility for patient care, providing for the patient’s health care needs and arranging for appropriate care with other qualified clinicians. The medical home is intended to result in more personalized, coordinated, effective and efficient care.


A medical home achieves these goals through a high level of accessibility, providing excellent communication among patients, clinicians and staff and taking full advantage of the latest information technology to prescribe, communicate, track test results, obtain clinical support information and monitor performance.



2012 Benefit Golf Tournament


Family Health Medical Services will be proud to start planning our 14th Annual Benefit Golf Tournament. The benefit will on June 13, 2012 at the Chautauqua Golf Club, Chautauqua, New York. All area golfers are invited to join us for the day. It will be a full day of fun in the sun and will be enjoyed by all.


Giving back to our community is a principle value at FHMS. In keeping with our commitment to support local organizations that directly benefit the quality of life in our community, we have chosen, again, to support the efforts of Hospice Chautauqua County in meeting their goal.


This endowment will ensure that those who have no insurance or who have exhausted their benefits can still receive the end of life care they deserve. Hospice, a nonprofit organization, provides compassionate medical treatment to support comfort and quality of life for patients and their families. Hospice staff provides a full range of social services, including nursing, education, bereavement counseling and pastoral care.
