Well Care and Routine Preventive Services
Our goal is to work with you in a Health Partnership to protect your health, and help you live as healthy as you can be. There are many diseases, illnesses and injuries that can be prevented by vaccination, early detection or other measures. Monitoring your health using the most up-to-date preventive health measures and clinical guidelines lets your doctor know how well you're doing and helps identify potential risks before there are complications.
If it's been a year since your last adult check-up, call for a complete preventive care exam. A comprehensive exam includes a complete physical, routine diagnostic lab tests, a gyn exam for women, and other exams, tests or procedures, depending on your age and health.
- Children are scheduled for periodic well-child exams to monitor their growth and development, and keep up with their immunizations
- Allergy shots, routine lab work, or blood pressure checks, are provided in between routine visits.
Immunization for Adults and Children
An immunization or vaccine is a medicine that's given to help prevent a disease. Vaccines help the body produce antibodies. These antibodies protect against the disease.
Vaccines not only help keep you and your child healthy, they help all of us stay healthy by reducing serious childhood diseases and the transmission of preventable diseases.
Chronic Disease Management
It's more important than ever those patients with long term health concerns work with their doctor to protect their health and prevent complications from their illness.
- Our providers encourage patients to learn as much as possible about managing their illness. Patients, receive important information at each visit about their progress toward personal goals.
- Chronic diseases like diabetes, asthma, heart disease and high blood pressure, and other serious long term illnesses, need frequent monitoring and evaluation to ensure that patients remain as healthy and productive as possible.
- Our diabetic and hypertensive patients are scheduled for return visits every six months, or more frequently, as needed. Routine diagnostic tests are ordered periodically to monitor the patient's progress and help the provider design the best treatment plan for the patient.
If you haven't thought about it in while, call today to schedule your annual check-up. Make sure you keep your six-month check-up; they are just as important to keep you healthy and on track.
Sick, Hospital and Emergency Care
- Sick Care - When you're sick, a call to the office early in the day will help us schedule you for a same-day or next-day visit. A triage nurse is available every day from 8:00am until 4:30pm to answer your questions. If necessary, she will forward important information to your doctor.
- Emergency Care - If you are experiencing a medical emergency or you believe you are experiencing a life threatening situation, call 911 immediately, or go to the emergency department of your nearest hospital. Our patients will be cared for by Emergency Room staff at all area hospitals and referred back to Family Health Medical Services for routine follow-up care.
- Hospital Care - When our patients need to be admitted to the hospital to monitor a serious medical situation, they will be admitted to either Brooks Memorial Hospital in Dunkirk, NY, or WCA Hospital in Jamestown, NY. Physician associates at those hospitals will provide care to our patients during their hospital stay, and refer our patients back to our practice for follow up care.
Pain Management
Our patients who experience chronic pain are enrolled into our Pain Management Program. Our goal is to work together with our patients to manage their pain so that patients can live as normal and productive a life as possible.
The provider will review the Pain Management Program that outlines the patients personal goals for pain reduction and medication use. Alternative strategies for the relief of pain are also reviewed. Patients will be asked to sign a Medication Use Agreement, describing the patient's rights and responsibilities in using medication for the pain management program.
Our patients meet monthly with their provider to review their pain management plan and, if necessary, renew their medication prescriptions.
Acupuncture
Dr. Berke is a trained and certified Acupuncturist. He uses acupuncture therapy as an alternative approach to smoking cessation, weight reduction, and pain management.
Acupuncture Therapy involves the placement of fine needles along energy pathways throughout the body to stimulate the unobstructed flow of energy and to influence overall health and well being. By eliminating congestion in these energy pathways, and activating the circulation of energy, pain can be relieved, diseases and symptoms alleviated, and addictive behaviors reduced.
Talk to your provider to see if acupuncture therapy is right for you. Acupuncture is not a replacement for conventional treatment, and your regular primary care provider should be consulted for all of your health care concerns.
Acupuncture is considered an alternative treatment and may not be covered by your health insurance. Contact your health insurance directly for information about your coverage options
Referral Specialist
Our Referral Specialist is dedicated to managing your provider's request for specialty care and diagnostic procedures that require your insurance companies authorization.
Requests for consultation with a specialist is first submitted to your insurance company, where required. Once authorization is received, the Referral Specialist will work with you to make arrangements for the appointment with the specialist, and transfer your medical and insurance information. Authorization and appointments are usually completed within a week of your provider's request.
Nursing Home Care
Routine health care is provided to our patients residing in long term care facilities:
- Absolut of Westfield
- Absolut of Dunkirk
- Lutheran Home and Rehab Center
We have expanded our patient services to Mayville Medical Arts Center, formerly The Gables Natural Medicine Center, in Mayville, NY.
Diagnostic studies can now be scheduled locally, including cardiovascular studies like echocardiograms and dopplers, nerve conduction studies, and dilated eye exams for diabetic patients. Nutrition infusions and chelation options are also available.
Research studies, in cooperation with national organizations, are available for patient participation. Current studies include researching effective treatments for heart disease and pain relief.
